The Hatto ("Sudden Awareness") Report - Use Stakeholder-Initiated CAPA to Drive Improvement

"Hatto" is Japanese for "recognition" or "sudden awareness," in the context of recognizing a potential risk or opportunity. The original context, "hiyari hatto," means "experience of almost accident situation" or occupational health and safety near-miss report. "Muda hatto" means recognition of waste, and "poka hatto" recognition of the chance of making an error (or, in general, a situation that might generate poor quality). The hatto (recognition or awareness repot) empowers frontline workers and other relevant interested parties to initiate corrective and preventive action (CAPA) for issues related to safety, quality, and waste. If the issue cannot be resolved by the process owner on the shop floor, the hatto report process hands it off to a formal CAPA process that uses a cross-functional team.
